The form title allows you to find which form you wish to edit in the dashboard and allow you to select it when adding it to your notice. You can also display the title above the form when adding it into a notice.

Adding a description to your form allows you to display any notes or information you need your parents to read before filling in the form.

You can add a description by tapping the Settings button in the top banner when creating or editing a form and then scrolling to the bottom of the page.
